New Patients

If you are a new patient being seen for the first time at Integrative Pediatrics of Ohio, LLC, please register through our patient portal. Once logged into the patient portal there will be a welcome email with instructions on the forms to complete and fax back to us at 614-855-8820. Once you have completed and returned the forms to us, we will contact you to schedule an appointment. If you have any questions, please feel free to contact us at 614-245-4750.

Appointment Fees/Insurance Information:

Due to the nature of providing integrative pediatric medical care, the cost of service at Integrative Pediatrics of Ohio, LLC depends on the complexity of the medical service needed to address a patient’s presenting problem(s). Dr. Allen T. Lewis, MD does not participate in any medical insurance plans; therefore, full payment is due at the time of the service. We accept cash, check, Visa, MasterCard, and Discover Card.

We can “courtesy bill” most insurances. This means we will need your insurance information, so that we can submit the medical insurance claim directly to your insurance company for you. The insurance company will then determine the benefits available to you and send the reimbursement check directly to you. Service provided by Dr. Allen T. Lewis, MD (DBA Integrative Pediatrics of Ohio, LLC) will typically fall under the “out of network providers” category.

Laboratory Testing Procedures:

If laboratory testing is required, Dr. Lewis will either provide a paper requisition or submit the order electronically. By arranging for laboratory testing in this manor, the laboratory will bill your insurance directly. Be sure to bring your insurance card with you to the laboratory appointment.

For most testing Dr. Lewis prefers the testing to be done at either Quest Diagnostics or ChildLab, as he has found their service to be consistently the most accurate and reliable.

Dr. Lewis may also order specialty testing with Genova Diagnostics, Inc., Doctor’s Data, Inc. or Direct Healthcare Access, Inc.  Billing for these services are arranged by these laboratories directly with the patient.

Supplement Purchases:

Nutrient supplements, educational materials, and other products are available for purchase at the time of service, on a walk-in basis, or by phone/fax order. These items are provided by IPO Bioscience, LLC and can be purchased with cash, check, Visa, MasterCard, and Discover Card.  Similar to other retail purchases, payment in full is due at the time of purchase.

Laboratory Testing Procedures

Preparation for testing:

Be sure to be well hydrated for the day of and the day before a blood draw. Be sure to bring your insurance card and the laboratory requisition, if a paper copy is needed. Please refrain from taking any supplements containing zinc for the day before and the day of the blood draw. If possible, refrain from taking over the counter anti-histamines for 2 weeks prior to the blood draw.

Blood and urine testing:

Laboratory testing can be done the same day after the patient appointment with Dr. Lewis, or can be arranged for a later date. Lab testing should be completed within 3 days of the appointment and must be completed within 2 weeks of the patient visit; otherwise, a second appointment will need to be scheduled to update the patient history. Testing completed after the 2 week mark will not be accepted without a repeat appointment.

For most testing, Dr. Lewis prefers the testing to be done at either LabCorp or Quest Diagnostics, as he has found their service to be consistently the most accurate and reliable. They participate in electronic ordering of laboratory testing, so no paperwork will be needed. A paper requisition or prescription will be provided for those patients as needed.

Testing with Doctor’s Data, Inc:

Testing with Direct Healthcare Access, Inc:
